I observed errors in pixel values and created this test to demonstrate the
problem.
The input is filled with pure blue 000003FF but when decoding it becomes
b=FFFE g=0000 r=001B
Any color is distorted somehow, not just pure blue.
How to reproduce:
Dont know best way to see pixels with command line, but this way it is
quite obvious in hex editor

I tested last ffmpeg with MSS2017. All samples from MSS are working
including h264->hevc conversion.
But ffmpegg doesn't work with hevc_qsv. I think, is uses other plugin
2fca99749fdb49aeb121a5b63ef568f7 than is needed?

the G726 codec ffmpeg implementation seems not to comply with ITU standard

* I'm trying to get video from 3 IP cameras and save locally as .flv (h264

* One of the 3 cameras starts failing, not dropping frames but stop

The `loudnorm` filter changes the sample rate of the input output stream,
sometimes doubling it, sometimes it will be even higher.
The desired behavior would be that the sample rate remains the same.
