40 ms desync is what you get when you are fifteen meter away from the scene.
That is something you experience rather frequently in real life when
watching shows or lectures with just basic sound reinforcement or none at
all, or talking to a neighbour through the window, playing in a field, etc.
I would say the brain is well trained to compensate that low a delay except
if you pay careful attention.

Actually, even in cinema theaters with surround speakers calibrated with
delay to account for the sound propagation, I would be surprised if the A-V
sync was much better than that for all seats. Maybe ~10 ms.
