I have one file (6 min) 250M in size, the problem occurs on ~4000 frame. It is 
1/6th of the file (~60M) 

To where should I put it?

In ffmpeg code there is it. Probably - in mpeg2video module, while reading an 
(error-containing) mpeg2 stream.
ffmpeg2theora only shows that that problem is.
