I did some checking at it turns out our FATE sample for qtrle-8bit uses 
the default palette handling code:

Agreed. It's also a shoehorning of another Microsoft format into a 
QuickTime file (MS RLE in QT). That might be the difference. Perhaps the 
palette needs to be unpacked in the opposite order.
