 It is, I agree, a matter of semantics - much of the document appears quite
 loosely written and several areas seem open to some degree of
 interpretation.

 My reading of "It is permitted to..." was that this sentence introduced an
 "exception" and that, by extension, the opposite to the exception should
 be considered the "rule".

 I apologise if I have made too much from those four words, it is not my
 intention to cause a problem just to find a reliable way of encoding
 chapters on the command line in a way that follows what most playback
 software seems to expect.

 I will try and follow the review of the patch (I'm not sure I understand
 much of the code!)
