If you had given me the file name, I would have 
found the sample ;-)

I tested the following:
$ ffmpeg -i fails.bin -qscale 2 out6.avi
$ ffmpeg -i fails.bin -qscale 2 -ac 2 out2.avi

Both output file play with perfect A/V sync 
afaict: I tested with mplayer -speed 0.7
