Where's the full output printed by ffmpeg when you run this command?  It
tells you, me, and everyone else lots of stuff.

I will guess that you don't have an H.264/AVC encoder in your ffmpeg
meaning you cannot make an mp4 file suitable for rubbish HTML5 video.
Recompile with libx264.
